New Patients

If you are requesting a new patient appointment, please include:

  • Your primary diagnosis or current symptoms

  • The reason you are seeking neurologic care

  • The name of your insurance plan

Please note: Your reason for consultation must fall within one of our specialty indications — chronic migraine, movement disorders, Parkinson’s disease, or essential tremor.

Providing complete and accurate information allows our team to review your request efficiently and determine appropriate scheduling.

All appointments are subject to clinical approval prior to confirmation.

Established Patients

If you are an established patient, please indicate whether your visit is for:

  • Routine follow-up

  • Medication management

  • Injection therapy

  • Infusion therapy

  • New or worsening symptoms

Please also confirm your current insurance coverage, as changes may affect scheduling and authorization requirements.
